Start with the Pennsylvania PIERS Public Portal.
Pennsylvania residents can use the secure PIERS Public Portal to request an official immunization record for themselves or a dependent.
Your record may not contain every vaccination you have received. A vaccine may be missing if it was not reported to Pennsylvania’s immunization registry or if the information in the registry does not match the information entered in the portal.
If you cannot find the record you need, Pennsylvania also provides an immunization record request process.
